responsibility for his or her own actions and conduct, follow safety and health procedures, and <br /> recognize and report hazards in their work areas. <br /> 4.1 Safety Inspection <br /> Tetra Tech will perform inspections to ensure that all personnel, including subcontractors are in <br /> compliance with this Health and Safety Plan. Inspections will include: <br /> • Means for inspecting, recording, and identifying safety and health deficiencies on a timely <br /> basis by authorized personnel and <br /> 0 Procedures for follow-up measures and inspections to ensure correction of deficiencies. <br /> 4.2 Emergency Response <br /> The SSHO will be responsible for initiating a medical and emergency service response. When <br /> emergency situations are handled on-site without the assistance of off-site personnel, it is the <br /> responsibility of site personnel involved to inform the SSHO immediately of the emergency at <br /> hand, no matter how insignificant it may seem to personnel involved. <br /> Should an accident occur, the SSHO notify the USACE PM, the project manager, and the <br /> PHSM, as appropriate. The SSHO will also complete an accident report (Attachment B) and <br /> investigate the cause of the accident. Any recommended hazard control measures will be <br /> discussed with the PHSM prior to implementation. Any chemical exposures or occupational <br /> injuries and illnesses will be reported and recorded.
