When
emergency situations are handled on-site without the assistance of off-site personnel, it is the
responsibility of site personnel involved to inform the SSHO immediately of the emergency at
hand, no matter how insignificant it may seem to personnel involved.
Should an accident occur, the SSHO notify the USACE PM, the project manager, and the
PHSM, as appropriate. The SSHO will also complete an accident report (Attachment B) and
investigate the cause of the accident. Any recommended hazard control measures will be
discussed with the PHSM prior to implementation.
