6.5 Monitoring Recordkeeping <br /> The SSHO will be responsible for establishing and maintaining records in a project logbook that <br /> will be available for inspection at all times. Required monitoring information is described <br /> below: <br /> • Employee name and task description; <br /> • The date, time, weather conditions, tasks being performed, and relevant comments <br /> will be written in a logbook that will be available for inspection at all times; and <br /> • Any engineering controls used to reduce exposure. <br /> 7.0 SITE CONTROL <br /> 7.1 Site Entry and Security <br /> A tailgate health and safety meeting for all contractor personnel will be conducted prior to site <br /> entry and commencement of work. The site will be visually inspected before task personnel <br /> enter the site. <br /> Daily control and accounting of all personnel present on-site will be accomplished by the <br /> following: <br /> • Daily Tailgate Safety Meeting form to be signed by all personnel present and working <br /> on the site; <br /> • Daily Visitor Log to be signed by all visitors to the site not providing a service to the <br /> project; and <br /> These forms are shown in Attachment C and will be maintained by the SSHO. <br /> 7.2 Site Communications <br /> Internal communication among on-site personnel has been described in Section 3.4 and in <br /> emergency situations, Section 3.8.2.
