well Permit Application Page 2 of 2 August 13,2010 Soil Sam"Im : Eight direct push soil borings will be advanced in the general area west of the subject property(See Figure 6).Each of the borings will be advanced to a total depth of 20 feet bgs using a direct push rig.The soil borings will be continuously cored for lithologic descriptions and final well construction design. A grab groundwater sample will be collected from each of the eight direct push borings using the screened sampler associated with the direct push rig.The sampler will be advanced to the target depth and retracted to expose the screen to groundwater•Apump and a riser tubing will be used to collect a groundwater samples in laboratory provided containers,labeled,placed on ice,and transported to a state certified analytical laboratory. It should be noted that for the 523 Larch Road property(APN#21220005),the Property owner requested that Bl,B2,B3, B4 and MW28 be relocated to the center of the property to avoid interferences with the flow of vehicular traffic. AA&A send an email request to the SJCDEH (copy attached) to relocated the borings and subsequently received approval from the SJCDEH(See attached approval letter dated 07-02-10). The boring and well locations depicted in the attached Site Map are as approved. Accordingly,Please find enclosed the following items for the above-referenced project: • A copy of the completed permit application for APN#21220004(3940 N.Tracy) • A copy of the completed permit application for APN#21220005 (523 Larch Road) • A copy of the Well Construction diagram&Site Map. • A copy of the San Joaquin County Parcel Map. • A copy of AA&A's Groundwater Investigation workplan. • A copy of the corresponding SJCDEH Workplan Approval Letter. • A copy of AA&A's email correspondence requesting relocation of borings and well,dated 06-28-10. • A copy of the corresponding SJCDEH Approval Letter. • A fully executed access agreement from the property owner(of APN#21220004)granting AA&A permission to drill on the property. • A fully executed access agreement from the property owner(of APN#21220005)granting AA&A permission to drill on the property. • A check in the amount of$868.00(made out to the San Joaquin County EHD)to cover the permit fees. Pennit fee breakdown: $89.00 per application+a fee of$345(3 hour mire num charge),thus the total cost for these two off-site properties amounts to$868.00.
