APP <br /> Well Installation and Sampling at California National Guard Field Maintenance Shop <br /> Stockton, California <br /> purchase orders. Workers will receive verbal and then written warnings if they fail to adhere to <br /> safety procedures and protocols established in this APP/SSHP and the USACE EM 385-1-1. <br /> • After one verbal warning of a major infraction (i.e. one that could lead to serious injury to <br /> the person or co-workers, the public, environmental releases or physical damage to the <br /> plant equipment) a second infraction is grounds for removal from the site. <br /> • For less serious infractions, a verbal and a written warning will be issued. However, <br /> another infraction of any type would be grounds for removal from the site. <br /> • Suppliers (i.e.: truck drivers) that are unsafe or fail to follow direction from site personnel <br /> will not be asked to return and their respective supervisors will be informed that they are <br /> no longer welcome at the site. <br /> 5.2 SAFETY RESPONSIBILITIES OF SUBCONTRACTORS AND SUPPLIERS <br /> The SSHO and PM will have the authority to make on-the-spot revisions to the APP/SSHP and <br /> stop work if an imminent hazard is discovered. Anyone seeing an imminent hazard, including <br /> the subcontractor personnel and suppliers arriving on site will also have the authority (and <br /> responsibility) to stop work if an imminent hazard is discovered. Work will resume only after the <br /> hazard has been ameliorated or eliminated. <br /> OTIE 11 <br />
