0
PUBLIC HEALTH SERV*ICES aaa .o
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
MEMO
TO: BRENDAN SHINE
FROM: MARGARET LAGORIO
DATE: APRIL 21, 1999
RE: LEPRINO FOODS, 2401 MACARTHUR, TRACY, CA
Billing forms have been previously completed by Leprino Food representatives.
Therefore, the items you need to submit to our agency in order to destroy
existing monitoring wells and install replacement wells, are a work plan, a permit
application from our agency (enclosed) and $89 permit fee per application for
well installation or $60 permit fee per application for well destruction. Verbal or
written approval of the work plan by the Central Valley Regional Water Quality
Control Board will have to be provided before we can issue the permit. Permits
are for parcels or sites so if wells are on different parcels or sites owned by
others, separate permit applications must be submitted. You may want to send
the work plan for review, so the number and type of permit applications can be
determined prior to the submittal of the permit applications and fees. I have
enclosed our well standards so you can be aware of them and know the work
must be done by a California licensed C-57 contractor.
A Division of San Joaquin Counry Healch Cam Services
