Hazardc us waste generators shall retain copies of all manifests signed off by the destination facility on site fc r three years and have them readily available for review. If the generator did not receive a copy of themanifest with the handwritten signature of the owner or operator of the facility to which the generator's w ste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the mani est within 45 days of the date that the waste was accepted by the initial transporter, the generatorshall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those eff rts. Exception reports shall be submitted to: DTSC Report Repo itory Generator Informati n Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit an exception report to DTSC. By October 13, 2012, submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception report submitted to DTSC. 126. The consolidated manifest#652467 from Evergreen Environmental Services was completed with the EPA ID# of a separate business. The consolidated manifest#606658 from Evergreen Environmental Services lacked a quantity for the amount of hazardous waste taken from the generator. The consolidated manifest#72884 from Evergreen Environmental Services lacked the active EPA ID# assigned to this business. All copies of each receipt shall contain all of the following information: (A) The name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator's representative. (B) The date of the shipment. (C) The manifest number. (D) The volume or quantity of each waste stream received, its California and RCRA waste codes, the wa to stream type listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North America (UN/UA) identification number, if applicable. (E) The name, address, and identification number of the authorized facility to which the hazardous waste will be transported. (F) The transporter's name, address, and identification number. (G) The driver's signature. (H) A statement, signed by the generator, certifying that the generator has ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
