The eyewash station i the boiler room was observed with chemicals and containers stored in front of it, blocking access. All fa iIity communications and alarm systems, fire protection equipment, spill control equipment, and deco tamination equipment, shall be tested and maintained as necessary to assure its proper operation i time of emergency. Immediately conduct all necessary testing and maintenance on this equipm nt, remove items stored in front of the eyewash, and maintain clear access to all decontamination and o her emergency equipment. REPEAT VIOLATION 404. A container of metal gri6dings was observed without a lid attached under the grinder in the shop (room S2). All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close th ese containers and ensure all hazardous waste containers are closed when not adding or removing wast . 605. A container of metal g dings was observed without a lid attached under the grinder in the shop (room S2). A 55 gallon blac steel drum of waste aerosol cans was observed in room S2 (shop) with a hazardous waste label witho t a hazardous property marked. All hazardous waste containers shall be marked with the following inf rmation: 1. the words "Hazardous aste" 2. name and address of g nerator 3. hazardous properties ( ammable, corrosive, reactive, toxic) 4. physical state (liquid, s lid) 5. composition (contents) 6. accumulation start date) Immediately label any container of hazardous waste metals and ensure that all containers are marked with all the required information. Since it cannot be determined how long the metal waste has been on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifes to the EHD by May 9, 2014. ALL EHD STAFF TIME ASSOCIATED ITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
