�Pu vF� <br /> I r <br /> � n <br /> m N <br /> I �BQB <br /> • December 8, 2004-Shell issued a letter clarifying the terms of the access agreement in <br /> Iquestion. <br /> • April 19, 2005—Clayton met with Bruce Kimbrough,proprietor of the dairy onsite, to <br /> discuss the upcoming activities. <br /> • April 25-April 26,2005 —Clayton excavatedl3 test pits to verify the locations of the <br /> former pipelines. In addition, Clayton tapped both of the pipelines in two locations (Test <br /> IPits 2 and 3) to verify that they no longer contained liquid. <br /> • May 9-May 23, 2005—Clayton performed a screening level soil and groundwater <br /> I investigation along the alignment. <br /> • July 11-July27, 2005—Shell retained Robert Heeley Inc. to expose, clean, cut, and <br /> remove the two inactive pipelines. Following removal of the lines, the pipeline trench <br /> Iwas backfilled. <br /> •: July 13-July 18, 2005—Clayton performed an additional soil and groundwater <br /> I investigation to further delineate areas where impacted soil was detected along the former <br /> pipelines' alignment. <br /> • August 22-August 30, 2005—Further investigation of impacted areas that were <br /> I • inaccessible during pipeline removal activities was completed. <br /> 4.0 PRE-FIELD ACTIVITIES <br /> Prior to field activities at the site, written access agreements from Mr. and Mrs. Machado were <br /> obtained. Clayton obtained permits for the proposed drilling activities from the San Joaquin <br /> County Environmental Health Department (SJCEHD)prior to fieldwork. As a requirement of <br /> obtaining a drilling permit, Clayton provided the SJCEHD with a copy of the access agreement. <br /> Clayton contacted Underground Service Alert (USA) 48 hours prior to drilling activities,to <br /> report our proposed excavation and drilling schedules and request the location of publicly owned <br /> utilities to be marked in the vicinity of the test pits and boring locations. <br /> Clayton prepared a Site Health and Safety Plan(SHSP) to reflect the work proposed at the site <br /> which detailed the work preformed, safety precautions, chemical exposure concerns, emergency <br /> I response procedures, nearest hospital location, and onsite personnel responsible for managing an <br /> emergency situation. A tailgate safety meeting was held onsite prior to commencing the work <br /> and included all personnel involved in the subsurface investigation. The safety plan was kept <br /> onsite during field activities. <br /> 5.0 INITIAL TEST PITS <br /> Prior to subsurface delineation activities, Clayton excavated test across the pipeline alignment in <br /> • order to verify the location of the pipelines.
