TASK-SY-TASK RISK ASSESSMENT <br /> JCompiete One Sheet for Each Task) <br /> TASK DESCRIPTION <br /> Task 2b — Installation of boreholes end/or groundwater monitor wells,installed to unknown depth below ground surface to <br /> characterize the presence of known impact to soil. Installation may be conducted with hand auger, geoprobe or with a hollow- <br /> stem auger drill rig, depending on depth. Soil samples will be collected with a hand sampler or split spoon sampler, and using a <br /> spoon or spatula placed into glass sample containers. Soil will be placed back into the borehole, unless not allowed then into <br /> drums. <br /> EQUIPMENT REQUIRED/USED <br /> (Be specific,e.g.;handtools.heavy equipmarrc, imtrummma, PPEI <br /> Drill rig type to be specified by task. Additional items include use of hand tools for drilling operations, monitoring with a OVM or <br /> HNU PID, and detection tubes (Draeger) , if necessary. PPE will include gloves (surgical and leather), steel toed boots, hard het, <br /> eye protection w/side shield and hearing protection (optional). Tyvek(optional) if muddy conditions. <br /> 'r POTENTIAL HAZARDSIRISKS <br /> CHEMICAL <br /> ® Hazard Present Risk Level: ❑ H ® M ❑ L <br /> What Justifies Risk Level? Intrusive activities bring subsurface materials to the ground surface + the drill rig generates heat <br /> which may volatilize organics in the soil. Monitoring with a PID, breathing zone and downhole, conducted during all intrusive <br /> activities, Draeger tubes for benzene (IF SUSPECTED)may be used if readings in breathing zone are sustained above 1 RRU <br /> (action level). Mod. Level D will be used unless benzene level indicate stop work or PID readings indicate need to elevate LOP. <br /> PHYSICAL <br /> ® Hazard Present Risk Level: ❑ H ® M ❑ L <br /> What Justifies Risk Level? Hand auger/Drilling operation pose potential for slip, trip, fall, noise, entanglement,pinch, cuts, <br /> bruises, encountering buried debris, and severity of amputation or death. Personnel will have training for and implement safe <br /> work practices during drill rig operations. Leather gloves will limit cuts and bruises. Good housekeeping will be implemented to <br /> reduce risk of slip, trip and fall hazards. Personnel will use hard hat, steel toed boots, safety glasses, and ear plugs (optional). <br /> BIOLOGICAL <br /> ® Hazard Present Risk Level: ❑ H ❑ M IN L <br /> What Justifies Risk Level? Work location is within an operational area that is covered with asphalt or weeds and little debris in <br /> the area where animals, rodents or snakes could be present. Personnel will be cognizant of the work location and any potential <br /> presence of hazards. <br /> RADIOLOGICAL <br /> ❑ Hazard Present Risk Level: ❑ H ❑ M ❑ L <br /> What Justifies Risk Level? None suspected. <br /> LEVELS OF PROTECTION/JUSTIFICATION <br /> Leval D and Modified Level D with cotton coveralls, eye protection, hard hat, steel toed boots, surgical gloves, leather gloves, and <br /> tyvek if operations become muddy or dusty. See discussions above for justification. Upgrade to Level C if PID exhibits 1 — <br /> S RRU sustained above background in breathing zone and if benzene (IF SUSPECTED) detected at less than 1 ppm with <br /> Draeger tubes.
