AIL <br /> TASK-BY-TASK RISK ASSESSMENT <br /> (Complete One Sheet for Each as <br /> TASK DESCRIPTION <br /> Task 2c — The groundwater monitor wells installed at the site will be developed and sampled using bailers, all purge water will <br /> be placed in drums or other suitable container. <br /> EQUIPMENT REQUIRED/USED <br /> (Se epecflic,e.4..hard tools.heavy equfprnettt,ktstrumerns,PPE) <br /> See above. Additional items include use of hand tools for well sampling. PPE will include gloves (surgical and leather), cotton <br /> coveralls, steel toed boots, eye protection w/side shield. Initial developing/sampling will include monitoring downhole and <br /> breathing zone with PID. If no elemental volatiles are detected during the initial sampling event, subsequent sampling events will <br /> not require the use of a PID. <br /> POTENTIAL HAZARDS/RISKS <br /> CHEMICAL <br /> ® Hazard Present Risk Level: ❑ H OM ❑ L <br /> What Justifies Risk Level? Well development/groundwater sampling pose the potential for contact with contaminated water. <br /> Monitoring with a PID will be conducted in the well headspace (initial sampling event) after removal of the well cap, personnel will <br /> stand in an upwind direction (all sampling events). Mod. Level D will be used unless PID readings in the breathing zone indicate <br /> the need to elevate level of protection. Surgical/nitrile gloves and eye protection will be used to protect against contact. <br /> PHYSICAL <br /> ® Hazard Present Rick Levee: 13H 13M OL <br /> What Justifies Risk Level? Well development and sampling activities pose the potential for cuts, bruises, slip, trip, or fall and <br /> muscle or back strain. Personnel will have training for and implement safe work practices during sampling activities. Good <br /> housekeeping will be implemented to reduce risk of slip, trip and fall hazards. <br /> BIOLOGICAL <br /> ® Hazard Present Risk Level: ❑ H OM OIL <br /> What Justifies Risk Level? Work location is within an operational area that is covered with asphalt and some weeds and little <br /> debris in the area where animals, rodents or snakes could be present. Personnel will be cognizant of the work location and any <br /> potential presence of hazards. <br /> RADIOLOGICAL <br /> ❑ Hazard Present Risk Levd: ❑ H ❑ M ❑ L <br /> What Justifies Risk Level? None suspected. <br /> LEVELS OF PROTECTIONIJUSTIFICATION'. - <br /> Level D and Modified Level D — eye protection, steel toed boots, surgical gloves, leather gloves, and tyvek if collect samples of <br /> product in wells. For initial sampling event: Upgrade to Level C, if PID exhibits 1 RRU sustained above background the <br /> breathing zone and if benzene (IF SUSPECTED)is detected at less than 1 ppm with Draeger tubes. Stop work if > ; RRU or <br /> > = 1 ppm benzene sustained in breathing zone.
