TASK-BY TASK RISK ASSESSMENT <br /> (Complete One Sheet for Each Task) <br /> TASK DESCRIPTION <br /> Task 2d — Surface water and sediment present at the site may include ponds, streams, sumps sampled with teflon dipper. Any <br /> decon waters generated will be placed in drums. Sampling is conducted with dipper and shovel. <br /> EQUIPMENT REQUIREDIUSED <br /> (Be specific,e.g.,hand tools:heavy equipment,instrumenna,PPE) <br /> See above. Additional items include use of hand tools for surface water/sediment sampling, monitoring with a OVM or HNU PID. <br /> PPE will include gloves (surgical and leather), cotton coveralls, steel toed boots, hard hat, eye protection w/side shield. <br /> POTENTIAL HAZARDS/RISKS - <br /> CHEMICAL <br /> ® Hazard Present Risk Level: 13 H ® M ❑ L <br /> What Justifies Risk Level? Surface water/sediment sampling pose the potential for contact with contaminated water. Monitoring <br /> with a PID will be conducted during sediment sampling by screening headspace in a jar or a bag containing a small amount of <br /> sediment. No monitoring is necessary for surface water sampling. Mod. Level D will be used unless PID readings indicate the <br /> need to elevate level of protection. Surgical/nitrile gloves and eye protection will be used to protect against contact. <br /> PHYSICAL <br /> ® Hazard Present Risk Level: ❑ H 0 M IN L <br /> What Justifies Risk Leve? Surface water and sediment sampling activities pose the potential for cuts, bruises, slip, trip, or fall <br /> and muscle or back strain. Personnel will have training for and implement safe work practices during sampling activities. Good <br /> housekeeping will be implemented to reduce risk of slip, trip and fall hazards. Caution will be used during any activities <br /> conducted along the shoreline or near water(slow flowing). <br /> BIOLOGICAL <br /> ® Hazard Present Risk Leve: ❑ H ❑ M ® L <br /> What Justifies Risk Level? Work location is within an area that is covered with some weeds and little debris in the area where <br /> animals, rodents or snakes could be present. Personnel will be cognizant of the work location and any potential presence of <br /> hazards. <br /> RADIOLOGICAL <br /> ❑ Hazard Present Risk Leve: ❑ H 11 M 13 L <br /> What Justifies Risk Level? None suspected. <br /> LEVELS OF PROTECTION/JUSTIFICATION <br /> Level D and Modified Level D with cotton coveralls, eye protection w/side shield, hard hat, steel toed boots, surgical gloves, <br /> leather gloves, and tyvek if operations become muddy or dusty.
