Any well destroyed shall be replaced <br /> under a permit issued by PHS-EHD immediately after <br /> completion of the remodeling. <br /> -3- Off-site investigation shall commence prior to, if not <br /> concurrently, with site remodeling. Zero lines of soil and <br /> groundwater contamination shall be defined and the PAR <br /> submitted within one year of site remodeling. <br /> -4- Private wells within a 2000 foot radius of the site shall be <br /> identified and sampled for TPH-G and BTEX constituents and <br /> the results shall be submitted in report form to PHS-EHD by <br /> April 25 , 1995 . The length of these well ' s screened <br /> intervals and their depth below surface grade shall be <br /> included in this report also. <br /> Private wells found to be contaminated with petroleum <br /> products shall be taken out of service immediately or <br /> adequate steps taken to return the well to drinking water <br /> standards . <br /> -5- Under no circumstances will the proposed remodeling of this <br /> station cause a discontinuation of any investigative or <br /> remedial activity, as required by PHS-EHD . <br />
