.�� <br /> 6.0 SAFETY REQUIREMENTS FOR PREPARATION OF HEALTH AND SAFETY PLAN (HSP) , <br /> SPECIFICATIONS AND SITE SAFETY AND HEALTH PLAN (SSHP) <br /> 6. 1 General. Each of the health and safety documents required by this <br /> SOW shall address the elements as described in this section. The level of <br /> detail provided shall be tailored to the type of work, complexity of opera- <br /> tions to be accomplished, and the hazards anticipated. Where a specific ele- <br /> ment is not applicable, make negative declaration in the plan to establish <br /> that adequate consideration was given the topic, and provide a brief jus- <br /> tification for its omission. The eventual construction/operations contractor <br /> shall be required to prepare and submit necessary amendments to the SSHP for <br /> approval prior to commencement of field work. <br /> 6.2 Staff Organization, Oualifications and Responsibilities. Discuss <br /> the organizational structure, including lines of authority for safety and <br /> health and overall responsibilities of the Implementation Contractor and any <br /> subcontractors for site activities including supervisor/employee relation- <br /> ships. An organizational chart showing the lines of authority for safety <br /> shall be provided. <br /> 6.3 Site Description. Provide a description of the site based on <br /> results of previous studies, site history and prior site uses and activities. <br /> Describe the location topography, and approximate size of the site, the on- <br /> site jobs/tasks to be performed and the duration of planned activities. Com- <br /> pile a summary of hazardous substances and safety and health hazards likely to <br /> be encountered on site. Include chemical/biological names, concentration <br /> ranges, media in which found, locations on-site and estimated <br /> quantities/volumes to be impacted by site work. If previous study reports are <br /> not available provide the necessary information based on prior site uses and <br /> activities. <br /> 6.4 Hazard Assessment and Risk Analysis. Identify site specific <br /> chemical, physical, safety and biological hazards that may be encountered for <br /> each task and/or site operation to be performed at the site. Each <br /> task/operation shall be discussed separately. Describe chemical, physical <br /> properties of selected contaminants, sources and pathways of employee ex- <br /> posure, anticipated on-site and off-site exposure potential levels, and the <br /> applicable regulatory standards and/or recommended protective exposure <br /> levels.
