Bernardino County Fire Department, Hazardous Materials Division and the State Office of Emergency
Services of the following:
Name of reporter
Telephone number of reporter
Name of facility _
Address of facility
Time of incident
Type of incident (e.g., fire)
Name of material(s) involved
Quantity of material(s) involved
Extent of injuries, if any
Possible hazards to health or environment outside facility
12. Prevention of spread or recurrence of emergency
During an emergency, Emergency Coordinator will take all reasonable measures necessary to ensure
fires, explosions and releases do not occur, recur, or spread. These measures will include stopping
processes and operations and collecting and containing released waste.
If a fire, explosion or release has occurred, the cause shall be identified and eliminated, if
possible.
13. Monitoring
If the facility stops operations in response to a fire, explosion, or release, the Emergency
Coordinator will monitor for leaks, pressure buildup, gas generation, or ruptures in valves and ancillary
equipment.
14. Provision for treatment, storage, or disposal
Immediately after an emergency, the Emergency Coordinator will make arrangements for treatment,
storage, or disposal of recovered wastes, contaminated soil, surface water, or any other contaminated
materials. Materials that result from a release, fire or explosion at the facility will be transferred into
appropriate tanks, containers, or transport vehicles and either stored until repairs are made or routed to
another facility.
Unless specifically determined to be otherwise, any spill clean up materials, absorbents, soiled rags,
gloves, etc. would be managed as hazardous wastes. Materials would be assumed to have all the same
hazard characteristics as waste received at the facility and managed accordingly.
Revised 3/1/2013 7
