Highway 99, Stockton, CA <br /> Dear Ms. Duncan: <br /> Ground Zero Analysis, Inc. (Ground Zero) is submitting a completed drilling permit application and <br /> associated fee for the destruction of nine (9) groundwater monitoring wells and one (1) groundwater <br /> extraction well, all of which are located at the subject site (Figure 1). The well destruction activities <br /> will be conducted in accordance with Ground Zero's Groundwater Monitoring Well Destruction <br /> Workplan, dated June 9, 2010, which was approved by the San Joaquin County Environmental <br /> Health Department (EHD) in correspondence dated June 15, 2010. A copy of the EHD Workplan <br /> approval letter has been included in Attachment A. <br /> Well destruction activities will be conducted by V&W Drilling, Inc. (C57 license #720904) <br /> under the supervision of a Ground Zero geologist. Groundwater monitoring wells MW5, MW7, <br /> MW8, MW 10, MW 11, MW 1413, MW 17B, MW 18C, and MW 19D and groundwater extraction <br /> well EW2 will be destroyed by pressure grouting with neat cement grout. Prior to destruction, the <br /> total depth of each well will be measured to ensure no obstructions are present. Neat cement <br /> grout will then be pumped into each well through tremie equipment in one continuous operation <br /> until each well is completely full. Additional cement will then be added under pressure to force <br /> the grout through the well screen and into the filterpack, and subsequently checked for <br /> subsidence. If the grout level is observed to have dropped, additional cement will be added <br /> under pressure until stable. <br /> Upon stabilization, the well monument and upper five feet of well casing will be removed from <br /> each well by overdrilling with a bit similarly sized to the originally drilled hole. The resulting <br /> annulus will be backfilled with neat cement grout to 3-feet below grade and the remaining <br /> annulus will be backfilled with native material to grade. Because the wells are all located in a <br /> field that is regularly disked the surface will remain dirt. In addition, the intermediate and deep <br /> zone monitoring wells were constructed with steel conducted casing, which will be removed at 4- <br /> feet below grade with a cutting torch, and prior to destruction of the extraction well (EW2) the <br /> GAGR0UNDZEITULEBERG1Permits&Applications1We11 Dest Permit Cvr.doc <br />
