B. Assess hazards to human health and the environment. <br /> C. Initiate appropriate response measures. <br /> D. Provide emergency notifications <br /> 9. If this spill is thought or known to be hazardous (drums, pools of unknown liquids, <br /> strong or dangerous odors, piles of unknown solids, dangerous powdered materials, the <br /> presence of dead or discolored plants or animals, etc.) the employee shall immediately <br /> take the following action: <br /> No-Smoking/No Fires <br /> A. Move a safe distance away from the area. <br /> B. Stay upwind, if possible. <br /> C. Do not handle any of the substances or materials or breathe any fumes. <br /> D. Do not attempt to collect any samples. <br /> E. Notify proper authorities <br /> (1) Call 911 <br /> (2) All numbers on contact forms <br /> * * * * F. Always allow the professional to have priority and do their job. <br /> 10. If an immediate threat to human life or health exists, call the National Response <br /> Center (NRC) at 1-800-424-8802. The NRC is manned by the United States Coast <br /> Guard can provide information responding to a spill. <br /> 11 . When the situation is under complete control and/or concluded, the Hazardous <br /> Waste Materials Coordinator at the project level shall document all necessary <br /> paperwork and forward it to the appropriate agencies and the Regional Hazardous <br /> Waste Coordinator. <br /> 12. The following information must be reported at the time of reporting: <br /> A. Facility identification <br /> B. Spill time and date <br /> C. Type of product spilled <br /> D. Estimated gallons spilled <br /> E. Whether waterway is affected <br /> F. Medium affected by Spill <br /> G. Damages for injuries caused by Spill <br /> H. Cause of spill <br /> I. Actions undertaken to stop, remove or mitigate the effects of this spill. <br /> J. Whether evacuation is needed <br /> K. Names of other persons and agencies contacted <br /> L. Direction of spill <br /> M. Identity of Facility Designated Accountable Person <br /> 60 <br />
