4. DESCRIPTION OF WORK TO BE CONDUCTED: <br /> The work to be conducted will consist of the following tasks. <br /> Task 1- Health and Safety Plan <br /> Develop a work plan and worker health and safety plan for the expanded <br /> soil and groundwater site assessment. Procedures for conducting all work <br /> are outlined in the Worker Health and Safety Plan prepared by Consultant <br /> (Attachment 2). Site-specific information is provided on the cover page of <br /> the worker health and safety plan. <br /> Task 2- Sensitive Receptor Survey <br /> Consultant will conduct a sensitive receptor survey within a 250-foot <br /> radius of the site. A vehicular reconnaissance of the site vicinity will be <br /> conducted to identify surface water bodies, schools, and potential water <br /> well users. A summary of the data will be presented in tabular form and <br /> will include information pertaining to the type of receptor, distance from <br /> the septic tank location, and if readily available, information regarding well <br /> screen intervals. <br /> Task 3- Groundwater Annual Monitoring <br /> Groundwater annual monitoring of the groundwater supply well onsite <br /> monitoring wells will be initiated until directed otherwise by SJCEHD. <br /> Attachment 3 describes the procedures for sampling the monitoring wells. <br /> Water samples will be analyzed using full scan USEPA Test Method 524.2 <br /> and 300.0 by a state of California certified analytical laboratory. <br /> Task 4- Subsurface Investigation <br /> Consultant proposes to collect soil samples to determine if petroleum <br /> hydrocarbon and/or volatile organic hydrocarbons-impacted soil is present <br /> beneath the site. The limited site assessment will include the following <br /> tasks: <br /> • Permitting and pre-field work activities; <br /> • Partial excavation of sampling locations; <br /> • Soil sample collection and analysis; and <br /> • Report preparation. <br /> Permitting and Pre-field Work Activities <br /> A site assessment boring permit will be obtained from the SJCEHD for all <br /> on-site boring locations. In addition, a health and safety plan will be <br /> prepared in accordance with Occupational Safety and Health Guidance <br /> Manual for Hazardous Waste Site Activities (National Institute for <br /> Occupational Safety and Health Administration, U.S. Coast Guard and <br /> U.S. Environmental Protection Agency, 1985). <br /> Main\D\Environmental\St.FrancisMotel\Workplan051005.wpd 7 <br />
