These re ords did not comply with requirements set forth in California Code of <br />Regulations, Title 22 (see violations). An incident log was faxed to my office later by Barry <br />Ballance. <br />IX. DISC <br />After reviewing available documents, I discussed the inspection with Mr. Dishman. I informed <br />him that I would be in touch with Debbie Blanchard with additional questions and that a <br />report would be set to her upon its completion. PHS-EHD informed Mr. Dishman and Mr. <br />Brennan that addit oval information and/or site visits may be required. They both agreed to <br />help with any futurp needs of PHS-EHD. With the inspection complete, I left the facility. <br />On March 16, 1994, I returned to the site to obtain more samples. I took a sample from the <br />floating sludge and the bottom sludge and submitted them under chain of custody to FGL <br />Laboratory for fish bioassay analysis. On April 4, 1994, I reviewed the analytical results and <br />found that the sample obtained from the bottom sludge failed the fish bioassay test and should <br />be considered haz dour by the toxicity criteria.
