The contingency plan <br />or this facility is outdated. The contingency plan on site was dated October <br />1, 2009 and did not contain <br />the most recent information. According to Mr. Exon, a current contingency <br />plan exists, but was not av ilable <br />on site. The contingency plan shall be reviewed, and immediately <br />amended, if necessary, wh <br />never any of the following occur: <br />- applicable regulations <br />re revised <br />- the plan fails in an em <br />rgency <br />- the facility changes in i <br />s design, construction, operation, maintenance, or other circumstance in a <br />way that materially inc <br />eases the potential for fires, explosions, or releases of hazardous waste or <br />hazardous waste cons <br />ituents, or changes the response necessary in an emergency <br />- the list of emergency c <br />ordinators changes <br />- the list of emergency e <br />uipment changes <br />Immediately amend the con <br />ingency plan for this facility to include the updated information, maintain a <br />current copy on site at all ti <br />es, and submit a copy of the updated contingency to the EHD by December <br />6, 2013. REPEAT VIOLATI <br />N <br />113. No emergency coordinator <br />has been identified for this facility. The facility has listed an emergency <br />call center number in lieu of <br />fan emergency coordinator. At all times there shall be at least one employee <br />either on the facility premise <br />or on call (available to respond to an emergency by reaching the facility <br />within a short period of time) <br />with the responsibility for coordinating all emergency response measures. <br />This emergency coordinator <br />shall be a person who is thoroughly familiar with- <br />- all aspects of the facility <br />s contingency plan (and have the authority to commit the resources needed <br />to carry out the conting <br />ncy plan) <br />- all operations and activi <br />ies at the facility <br />- the location and charact <br />ristics of waste handled <br />- the location of all record <br />within the facility <br />- the facility layout <br />Immediately appoint an eme <br />gency coordinator for this facility. <br />117. The training recordson <br />site were incomplete. The training records do not include the names and <br />job titles of each employee, <br />description of the training to be given to each employee, or a record that <br />every employee received th <br />training. Each employee has access to their own training records, <br />indicating which trainings the <br />have received, but no records were available to demonstrate that training <br />was completed for all emplo <br />ees. Facility personnel shall complete a training program that teaches <br />them to perform their duties <br />i a way that ensures the facility's compliance with Title 22 regulations.
