Manifest n mbers 009565024JJK (3/19/12), 009895723JJK (10/5/12), 010855822JJK (1/8/13), 009575261JJK (4/ /13), and 009897933JJK (7/2/13) were found without a signed off copy from the destination facility. Ha rdous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the ma ifest with the handwritten signature of the owner or operator of the facility to which the generator's was e was submitted within 35 days of the date the waste was accepted by the initial transporter, the gener for shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manif st within 45 days of the date that the waste was accepted by the initial transporter, the generator sh 11 submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter igned by the generator explaining the efforts to locate the hazardous waste and the results of those effo s.
