The emergency coor inator information in the contingency plan for this facility is outdated. The contingency plan shall be eviewed, and immediately amended, if necessary; whenever any of the following occur: - applicable regulations are revised - the plan fails in an em rgency - the facility changes in is design, construction, operation, maintenance, or other circumstance in a way that materially inc eases the potential for fires, explosions, or releases of hazardous waste or hazardous waste cons ituents, or changes the response necessary in an emergency - the list of emergency oordinators changes - the list of emergency quipment changes Immediately amend the co tingency plan for this facility to include the updated information, maintain a current copy on site at all ti es, and submit a copy of the updated contingency to the EHD by February 27, 2012. 122, 123, & 124. Manifest umbers 005720578JJK (3/23/10), 008142922JJK (5/5/11), 008820883JJK (8/8/11), 008815800JJK (9 14/11), and 008133621JJK (11/11/11) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility o site for three years and have them readily available for review. If the generator did not receive a Fopy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy bf the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generl�tor shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Reposi cry Generator Informatio Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a opy of the missing manifests or prepare and submit an exception report to DTSC. By February 27, 2012, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. ALL EHD STAFF TIME ASSOCIATE WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE.
