<br />o the possible hazards to human health or environment outside the Facility
<br />• Take all reasonable measures to ensure that fires, explosions, and releases do not occur, recur, or spread to other hazardous
<br />wastes at the Facility.
<br />• Document all actions and mplete appropriate reports and notifications, including noting the following information in the
<br />Facility operating record: e, dafe, and details of the incident that required implementation of the Plan.
<br />• Within 15 days after the ' ident, a written report must be submitted to the designated state authority including the following
<br />information:
<br />o name, address, andltelephone number of the operator
<br />o name, address, anOelephone number of the Facility
<br />o date, time, and type of incident (e.g., fire, explosion)
<br />o name and quantity of material(s) involved
<br />o the extent of injuries, if known
<br />o an assessment of acIIItual or potential hazards to human or the environment
<br />o estimated quantity end disposition of recovered material that resulted from the incident
<br />Immediately after the emergency, I�a emergency coordinator will provide for treating, storing, or disposing of the recovered wastes,
<br />contaminated soil, or surface wate�, or any other material that resulted from the release, fire, or explosion at the Facility.
<br />Prior to restarting operations at thel Facility, the primary emergency coordinator will take appropriate steps to assure:
<br />• No waste that may_ be incoTpatible with the released material is staged or managed in the area of the incident until cleanup
<br />procedures are completed.
<br />• All emergency equipment listed in the Contingency Plan is cleaned and fit for its intended use. Appropriate equipment must be
<br />available in the event of an they incident.
<br />• Notify the Regional Administrator and appropriate State or local authority that the Facility is ready to resume operation.
<br />Action Plan - Hazardous Waste Spills
<br />In general, most facility personnel will not be responsible for participating in the emergency response to a release of a hazardous waste
<br />unless the spill is small, is limited t the immediate work area, the hazards associated with the waste are known, appropriate personnel
<br />protective equipment is available, d it is possible to clean up the spill promptly and safely.
<br />First Responder/Coordinator of *e Area: Should a spill of hazardous waste occur, the following actions should be taken:
<br />1. Note the location of spill, type and amount of material spilled, any injuries and the potential threat to human health or the
<br />
