ON 5/26 WAS TAKEN TO ST. JOSEPHS HOSPITAL BY AMBULANCE FROM PUBLIC LAUNDRY MAT AFTER <br /> MORE VOMITING AND PASSING OUT. SEEN DR. LEAVITT AND WAS TOLD COMPLAINANT HAD FOOD POISONING <br /> AND WAS GIVEN MEDICINE. Complainant stated subsequently in a phone call before the inspection that he had bacon <br /> cheese burger and banana milkshake. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #7 Hot and Cold Holding Temperatures <br /> OBSERVATIONS:Potentially hazardous food (PHF)above 41 F(see description below). Provide 41 F or below for all cold <br /> PHF, and 135 F for all hot PHF. <br /> CALCODE DESCRIPTION:Potentially hazardous foods shall be held at or below 41/45°F or at or above 135°F. (113996, 113998, <br /> 114037, 114343(a)) <br /> #8 Use of Time as a Public Health Control <br /> OBSERVATIONS:No initial time on container or log for the PHF in the 2D prep table (see below narrative). Label with initial <br /> time of PHF that uses time control only on container or in a separate log. Owner agreed to dispose referenced PHF within 4 <br /> hours from when they were first moved to the 2D prep table (see time listed above). <br /> CALCODE DESCRIPTION:When time only,rather than time and temperature is used as a public health control,records and <br /> documentation must be maintained(114000) <br /> #14 Food Contact Surfaces Sanitized or Warewashing Sanitization <br /> OBSERVATIONS:Employee failed to sanitize when asked to perform warewashing as a demonstration; owner stated that <br /> is done but employee had forgotten during demonstation. Ensure sanitizing by 100 ppm chlorine or 200 ppm quat is part of <br /> warewashing. <br /> CALCODE DESCRIPTION:All food contact surfaces of utensils and equipment shall be clean and sanitized.
