Use of Equipment <br /> Any equipment, including earth moving equipment, drill rigs, cranes, or <br /> other heavy machinery will be operated in strict compliance with the <br /> manufacturer's instructions, specifications, and limitations as well as any <br /> applicable regulations. The operator is responsible for inspecting the <br /> equipment daily to ensure that it is functioning properly and safely. This <br /> inspection will include all pins, pulleys, and connections subject to faster <br /> than normal wear and all lubrication points. <br /> When equipment with moving booms, arms, or masts are operated in the <br /> vicinity of overhead hazards, the operator, with assistance from the <br /> designated signaling person, will ensure that the moving parts of the <br /> equipment maintain safe clearances to the hazards. Equipment will be <br /> kept away from energized electrical lines by at least 20 feet. Drill rigs and <br /> other equipment not specifically designed to move with the boom, mast or <br /> arm elevated will be returned to traveling position and condition before <br /> being moved. <br /> All portable equipment and tools will be inspected prior to each day's use <br /> and as often as necessary to ensure that it is safe to use. Defective <br /> equipment and tools will be removed from service immediately. <br /> Examples of defective tools include: hooks and chains stretched beyond <br /> allowable deformations, cables and ropes with more than the allowable <br /> number of broken strands, missing grounding prongs on power tools, <br /> defective on/off switches, mushroomed heads of impact tools, sprung <br /> wrench jaws, missing or broken handles or guards as well as wooden <br /> handles which are cracked, splintered, or loose. All equipment and tools <br /> will be used within their rated capacities and capabilities. <br /> Excavation Safety <br /> Prior to excavating or trenching five feet deep or more into which a <br /> person is required to descend, a permit from the Division of Occupational <br /> Safety and Health (DOSH) is required. <br /> The location of all underground pipes, electrical conductors, fuel lines, <br /> water and sewer lines must be determined prior to any excavation work. <br /> All lines must be deenergize, locked-out or blinded off where feasible. <br /> The walls and spaces of all excavations which will be entered by personnel <br /> more than five feet deep shall be guarded by shoring, sloping or benching. <br /> Side slopes shall not be steeper than 3/4:1 without a written report from a <br /> qualified civil or geotechnical engineer. All excavations shall be in <br /> 4-4 <br />
