Page three <br /> Unless otherwise shown on the plans or specified herein, all excess materials <br /> (except drill cuttings) shall become property of the Contractor and disposed of by <br /> the Contractor. The City will accept the drill cuttings from the Lewis`iVlanor� <br /> Production and Monitoring Wells upon the Contractor's delivery of them to the' <br /> Waste Water Treatment Plant located on Arbor Road. The Contractor.shall, <br /> contact Frank Motzkus (WWTP Supervisor) at (209) 831-44.87 to„coordinate; <br /> delivery. The Contractor shall be responsible for disposing all excess materials <br /> in a safe and legal manner. No material shall be placed on private property or <br /> public property without prior approval from the City and the property owner. The <br /> Contractor shall not allow any refuse, excavated material, surplus concrete or <br /> mortar, or any washings therefrom, to be disposed upon paved streets, into <br /> catch basins, or otherwise into the City storm drain system. <br /> ITEM NO. 8 <br /> The first paragraph of DIVISION 11, Section 20 - `INSPECTION", shown on <br /> page 9, is hereby amended to read: <br /> The work shall be done by the Contractor to the complete satisfaction of the <br /> Engineer, and in accordance with the requirements of the City. Inspection of the <br /> work will be performed by the City, along with the permitting agencies (the" <br /> State's Department of Health Services and the County's Public Health-Services)'_ <br /> that have jurisdictional responsibilities over,a portion of the work. <br /> ITEM NO. 9 <br /> This item clarifies the Scope of Work to require the pilot hole on the <br /> Monitoring Well rather than the Production Well. All the necessary <br /> sampling data shall be obtained from this pilot hole. <br /> The "SCOPE OF WORK”described in DIVISION 111, Section 9 - "LEWIS <br /> MANOR WATER WELL AND NESTED MONITORING WELL DESIGN AND <br /> CONSTRUCTION", shown on pages 10, 11, and 12, is hereby amended to <br /> read: <br /> 1.02 SCOPE OF WORK <br /> The Contractor shall construct'a Production Well and Nested Monitoring'WNell as <br /> described below. In general, the sequence of construction will be to construct the pilot <br /> hole for the Monitoring Well(not the Production Well), conduct all logging, sampling and, <br /> analysis, ream the pilot hole, construct the Nested Monitoring Well, and construct the <br /> Production Well. <br /> A. Production Well. (Bid Item No. 1) <br /> 1. The work includes the complete construction of a production water <br /> supply well that is gravel packed and provided with well casing and <br /> screen sections, developed and tested.
