A corrected MRP will be submitted by our contractor (Belshire Environmental Services) <br /> under a separate cover(Ticket#9409262). <br /> • CCR 2712(b): Maintenance &monitoring records—On May 9, 2008,there was a <br /> low liquid alarm for sensor L-12 labeled as 92 annular. The designated operator <br /> noted there was an alarm & wrote maintenance invoice attached. The invoice <br /> attached had a note to all w/o 9079066. W/o 9079066 shows the blots were <br /> sheared/broken &needed to be replaced. Neither document explains how the <br /> alarm was responded or the correction action taken for the alarm. Maintenance <br /> records must be maintained for correction action& designated operators must <br /> verify that each alarm condition was documented&responded to appropriately. <br /> Win 30 days (10.17.08) submit to this office written documentation explaining <br /> how the alarm was responded to. <br /> The Inspection Reportstates that the May 2008 Monthly DO report was observed to be <br /> deficient. <br /> On May 9, 2008, our testing contractor(Tanknology)was onsite to complete compliance <br /> tests (tank and line tightness tests) (Ticket#9032570). During the testing event, the <br /> alarm (L-12)was triggered. Prior to leaving the site,the technician addressed any alarms <br /> at the site. I have attached a copy of the work order from the testing event (Attachment <br /> B). <br /> • HSC 25291(b): UST system monitoring equipment is operational—UST system <br /> monitoring equipment is operational HSC 25291(b). At time of inspection, <br /> V/R322 sensor in the 87 fill sump was not functioning. Technician onsite <br /> replaced w/a functional sensor. All monitoring equipment must be operational at <br /> all times. Immediately obtain a permit from this office for the work performed. <br /> A permit application must be submitted to this by the next business day or a <br /> double permit fee will be charged. <br /> The Inspection Report states that at the time of the inspection,the sensor in the unleaded <br /> underground storage tank(UST) failed during the compliance testing event. <br /> The technician from Tanknology replaced the sensor and retested the installed sensor <br /> with passing results. The permit application for this installed sensor was submitted to <br /> San Joaquin County Environmental Health Department by Tanknology on September 17, <br /> 2008. The increased fees will be submitted by Tanknology. <br /> • CCR 2715: Designated Operator(a) Statement from UST owner—Statement from <br /> UST owner CCR 2715(a). For the month of September, Lee Chang performed <br /> the monitoring inspection(D.O.) at this facility. At time of inspection, Lee Chang <br /> is not currently listed as the DO for this site or as an alternate w/in 30 days <br /> (10.17.08). Submit an updated designated operator list for this facility. <br /> The Inspection Report states that at the time of the inspection,the Designated UST <br /> Operator notification for the site was observed to be deficient. <br />
