In view of the person after he has taken <br /> possession. <br /> • Secured by that person so that no one can tamper <br /> with the sample without his knowledge. <br /> • Secured under lock and key by that person in an <br /> area which is restricted to unauthorized personnel. <br /> 3 . 3 CHAIN-OF-CUSTODY RECORD <br /> 3 . 3 . 1 A Chain-of-Custody record must be filled out and <br /> accompany each sample from its origin to its final <br /> destination. Figure 2 shows an example of the Chain- <br /> of-Custody record used. The document is combined <br /> with the Request-for-Analysis in a common form, <br /> Figure 2 . <br /> 3 . 3 . 2 Record one or more samples which have identical <br /> destination and handling on the Chain-of-Custody. <br /> Retain a photocopy of each completed form for lab <br /> records. <br /> 3 . 3 . 3 Every Chain-of-Custody must show the following <br /> information: <br /> • Collector ' s sample ID number , which must be a <br /> neutral number . <br /> • Number of containers of each sample. <br /> . Type of sample and field information. <br /> • Signature of sample collector . <br />
