a <br /> Domestic Well Installation Report(DW-211) 2 <br /> Ranch Market 23569 South Santa Fe Road,Riverbank CA Febru 1 2001 <br /> REGULATORY DIRECTION <br /> Replacement of the domestic well on the Pantoja Property was required by the San Joaquin <br /> County Environmental Health Division (SJCEHD) in letters dated April 26, 2000 and July 31, <br /> 2000 1n addition, the final well installation plan was reviewed and approved through meetings <br /> and discussions with the SJCEHD, the California Regional Water Quality Control Board, Central <br /> Valley Region (RWQCB), and the State Underground Storage Tank Cleanup Fund (USTCF) <br /> No formal well installation work plan was required by the RWQCB or the SJCEHD <br /> DOMESTIC WELL INSTALLATION <br /> Site Specific Health and Safety Plan <br /> SECOR completed the proposed work following the protocol established in the existing site- <br /> specific Health and Safety Plan, as required by the Occupational Health and Safety <br /> Administration (OSHA) Standard "Hazardous Waste Operations and Emergency Response" <br /> guidelines (29 CFR 1910 120) The document was reviewed and signed by all SECOR personnel <br /> and subcontractors pnor to performing work at the site <br /> Permitting/Utility Clearance <br /> A permit for well installation (Attachment 3) was obtained from the SJCEHD The required <br /> permitting was completed prior to performing the fieldwork Prior to conducting subsurface <br /> work at the site, Underground Service Alert (USA) was contacted to delineate subsurface piping <br /> in the area surrounding the proposed work area <br /> Well Installation <br /> On December 20, 2000, SECOR conducted the tailgate safety meeting with Bradley and Sons <br /> Drilling (Bradley) and observed the first day of drilling domestic well DW-2R From December <br /> 20 to December 22, 2000, Bradley used a mud-rotary drill rig to drill the boring from grade to <br /> 274 feet bgs On December 22, 2000, after Bradley finished the borehole, SECOR's field <br /> geologist observed Newman Well Survey run and record a resistivity and spontaneous potential <br /> log of the borehole from the total depth to the boring at 274 feet bgs to 30 feet bgs (Attachment <br /> 4) Bradley used the log to select and confirm where to set the well screen and blank casing <br /> Details of the lithology are described in the DWR Well Completion Report (Attachment 4) <br /> Copies of the field notes are included in Attachment 5 <br /> File DW 2 Destruct doc <br />
