2. SCOPE OF WORK <br /> CKG will contract with Gregg Drilling,a C57 licensed drilling contractor to install the new MW-5 at <br /> its present location.The following bullets outline the elements necessary to complete the proposed <br /> work: <br /> • Obtain a SJDEH Permit for the well installation <br /> • Prepare a site-specific health and safety plan for the work. <br /> • Mobilize to the site to install the monitoring well. <br /> • Develop and survey the monitoring well. <br /> The following is a task breakdown of CKG's scope of work. <br /> Task 1 Permitting <br /> The SJDEH requires submittal of their"Site Mitigation Well and Boring Permit Application". <br /> Supporting documents require3d with the permit applications include a site-specific health and safety <br /> plan;their"Site Mitigation Master File Record Information Form"and this work plan,approved by <br /> the RWQCB. Copies of these document are provided in Appendix A. <br /> Task 2—Health and Safety Plan <br /> A health and safety plan will be prepared prior to initiating fieldwork. The health and safety plan will <br /> list potential hazards and protective measures required to protect the field investigation team,other <br /> persons on site,and the general public from site specific hazards(Appendix A). <br /> Task 3-Subsurface Utility Clearance <br /> Because the new MW-5 will be installed in the same borehole as the old MW-5,it will not be <br /> necessary to clear the borehole for subsurface utilities. <br /> Task 4—Well Installation <br /> CKG will work with Gregg Drilling to install the new MW-5 (MW-5R)to a depth of 85 feet which is <br /> approximately 10 feet deeper that the existing MW-5.The well will be constructed through 8-inch <br /> diameter hollow stem augurs.The well will be constructed of 2-inch diameter schedule 40 PVC with <br /> 20 feet of 0.02-inch screen from 65-85 feet below ground surface. <br /> Gregg Drilling will start by removing the flush mounted well head and drilling out the existing well. <br /> After all the will debris is cleared from the borehole the drillers will advance the hole to the new depth <br /> of 85 feet below ground surface.CKG Environmental,Inc.will log soil lithology from the deeper <br /> section of the well.Construction details for MW-5R are described on Plate 3. <br /> After the borehole has been drilled the well casing will be placed into the augurs and the sand pack <br /> material placed in the annular space around the screened interval.
