1 <br /> • Notify the HS Department when field operations begin so that field support can <br /> be scheduled; <br /> • Ensure that the HASP is read and signed by aIl field personnel on the project, <br /> including subcontractors. HASP must also be signed by HS Manager and the <br /> ' PM, <br /> • Ensure that all provisions of the HASP are followed Contact the HS <br /> Department for any variances or modifications desired, <br /> ' • Demonstrate a personal commitment to safety on the project, <br /> • Ensure that tailgate safety meetings are conducted daily, signed by all field <br /> workers and reviewed by the FC and the PM The FC must have completed the <br /> ' Hazardous Waste Supervisor Course, <br /> • Ensure that Field Activity Daily Log (FADL) forms are completed for each day <br /> of operations, signed and dated by the author, and that all persons listed have <br /> ' signed the HASP and tailgate form; <br /> • Have supervisors inspect the project at least weekly, with inspections and <br /> corrective actions documented on FADL forms. PMs inspect projects <br /> periodically, <br /> • Ensure correction of any reported or observed safety hazard; <br /> • Ensure employees are trained on the hazards of any hazardous substances (other <br /> than wastes) used MSDSs must be on hand and containers must be properly <br /> labelled, <br /> • Ensure that project safety equipment is inspected regularly (monthly for fine <br /> extinguishers); <br /> • Report all near-miss, injury, illness and vehicle accident incidents to the HS <br /> Department within 24 hours and ensure that a Supervisors Employee Injury <br />' Report (SEIR) form is initiated, <br /> • Notify the HS Department when field work lasts more than six months so that <br /> the HASP can be reviewed and updated as needed; <br />' • Immediately notify the HS Department upon receiving notice of any OSHA <br /> inspection, and <br /> • Ensure that the project files receive copies of- <br /> - <br /> f- all internal and external HS correspondence <br />' - all air sampling records (including "none-detected") <br /> - all accident reports and Accident Review Board documentation <br /> - documentation of audits and corrective actions <br /> - air monitoring equipment calibration records <br /> - all FADLs. <br /> The PM will lead at least one site safety audit team per quarter, while field activities are <br /> conducted and will ensure that all accidents, incidents and/or near-misses are investigated in a <br />' timely manner.
