1601 Turnpike <br /> Page 2 of 2 <br /> COMMENTS AND CONDITIONS: <br /> 1. Submit a boring permit application and $374.00 permit and inspection fee to San Joaquin <br /> County Environmental Health Department,(EHD). <br /> 2. Notify the EHD at least 72 hours prior to commencement of fieldwork. <br /> 3. Report groundwater samples for 1,2-DCA and EDB at 0.5 micrograms per liter(Ag/L), if <br /> Possible. <br /> 4. Drilling cuttings generated during the well installations will be stored in Department of <br /> Transportation-approved 55-gallon drums, characterized, and properly disposed of at an <br /> appropriate landfill. The drums will be removed within 90 days of installation of the <br /> wells. <br /> 5. Complete the fieldwork consisting of well installations, development, surveying, and <br /> initial quarterly groundwater monitoring by November 20,2006. <br /> 6.
