The CHP must be notified of a spill or release of any hazardous material/waste in a Caltrans <br /> maintenance facility that requires response by outside agencies or private resources. <br /> NOTIFICATION <br /> In the event you are required to respond to a known or suspected spill or release (regardless of <br /> size) involving a hazardous material, the event shall be reported immediately to the Traffic <br /> Management Center Dispatch(TMC). <br /> TMC will be responsible for notifying District staff personnel and for making all required <br /> notifications outside of Caltrans in accordance with Appendix A. TMC dispatch will notify the <br /> Supervisor, Area Superintendent and Region Manager as soon as the situation permits. <br /> REPORTING <br /> It is the responsibility of the Supervisor to enter all spill information into the Integrated <br /> Maintenance Management System (IMMS). Unless directed otherwise, the FRO'will do the <br /> following to document the spill: <br /> (1) Attempt to secure responsible party identification from CHP or the local police <br /> That is the IC on scene. <br /> (2) Complete a Spilled Substance Report and enter information into the <br /> Spill Info tab on the IMMS Service Request form. <br /> (3) Prepare a Spill Narrative and enter information on the Service Request comments <br /> Tab. <br /> (4) Complete a Hazardous Spill Diary Form. <br /> (5) Ensure that a Unifonn Hazardous Waste Manifest is prepared. <br /> (6) Note the assigned IMMS Service Request number on the Spill Diary Form <br /> And complete all required data fields in IMMS. <br /> (7) Ensure a Site Safety Plan is prepared prior to initiating cleanup operations. <br /> *Original documents to District 10 Hazardous Material Manager and copies to Region Office. <br /> 6 <br />
