B St., Stockton <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />(CLASS 1, CLASS II, or MINOR -Notice to Comply) <br />7. (continued) Submit a statement and supporting documentation explaining how this waste was <br />managed by July 29, 2011. <br />45. The contingency plan for this facility did not include descriptions of a list of emergency equipment at <br />the facility. A contingency plan shall include all of the following: <br />-actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br />or surface water <br />-arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br />& local emergency response teams <br />-names, addresses & phone numbers (office & home) of the Emergency Coordinator <br />-list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br />location, physical description, and a brief outline of its capabilities <br />-evacuation plan, including routes and signals to be used <br />-telephone number of Cal EMA <br />This information must be kept current at all times. Immediately update the contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />July 29, 2011. <br />49. A written training program and training records were not available on site. Facility personnel shall <br />complete a training program in accordance with Title 22 regulations. This program shall be directed by a <br />person trained in hazardous waste management procedures and shall include the following: <br />- hazardous waste management procedures, including contingency plan implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />The training program shall be completed by each employee within six months of hire, prior to <br />unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br />closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each position (job title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY ME AT URRENT HOURLY RATE. <br />EHD Inspector: <br />Received By: / <br />Tutee: <br />EHD 2 2-003 _ 08110/10 , CONTINUATION FORM <br />
