The regional administrator of <br /> EPA (Region IX) and appropriate State and -local authorities <br /> must be notified that thereleasedmaterial was treated, <br /> stored, or disposed of properly as a hazardous material <br /> and that all emergency equipment involved in. the contain- <br /> ment of the released material has been properly cleaned <br /> and is fit for its intended use. <br /> 0 The owner or operator must note in the operating record <br /> the time, date, and details of any incident that required <br /> implementing the contingency plan. Whithin 15 days after <br /> the incident, the .owner must submit a written report on <br /> the incident, on any. spill Of 1 gallon or more in quantity, <br /> to the California DOHS, Hazardous Waste Management Branch, <br /> and the EPA Region IX Administrator.— The report must <br /> include: <br /> Name, address, and telephone number of the owner <br /> or operator <br /> Name, address, and telephone niumber ' o-f the facility <br /> Date, time, and type of incident (e.t. ,fire, explosion) . <br /> Nameand quantity of material (s) involved <br /> - The extent of injuries, if any <br /> - An assessment of actual or potential hazards to <br /> human health or the environment, where this is <br /> applicable <br /> - Estimated quantity and disposition of recovered material <br /> that resulted from the incident <br /> x-6 <br />
