o Within 24 hours a complete investigation is conducted by the <br /> superintendent, foreman and safety specialist. When available the <br /> divisional vice president will also assist with the investigation. <br /> o Within 72 hours the Mobile Equipment Safety Advisory Committee <br /> convenes. At this time the employee is offered an opportunity to <br /> attend where the contributing factors leading to the accident or <br /> incident are reviewed. The purpose of this meeting is to: <br /> o Research and decide upon the root cause of the accident or <br /> incident. <br /> o Communicate to all employees what the findings are and to <br /> share as well as ask for their ideas as to how we can <br /> eliminate the accident/incident risk. <br /> o Decide if there is a disciplinary action that is to be taken if <br /> there was an unsafe action taken by the employee. If an <br /> action is to be taken the superintendent implements and the <br /> action is noted in the divisional safety disciplinary action <br /> database. <br /> SMITH DEFENSIVE DRIVING PROGRAM <br /> • The division safety specialist was certified and has trained and certified <br /> over 100 employees of our division as well as corporate personnel in the <br /> defensive driving program. <br /> o The purpose of the program is to teach defensive driving skills <br /> designed to make the driver more aware and more anticipatory of <br /> potential driving dangers. The trained driver is much more capable <br /> of seeing potential driving hazards and taking evasive action to <br /> eliminate the potential risk. <br /> o The program involves classroom presentation and training as well <br /> as hands on demonstration by the instructor and student of the <br /> principles of the defensive driving program. <br /> o The program has reduced our vehicle accidents by the following: <br /> o 2000 to 2006 Vehicle Accident Average = 6 <br /> o 2007 to 2010 Vehicle Accident Average = 2.5 <br /> o 2006 to 2006 Vehicle At Fault Accident Average = 2 <br /> o 2007 to 2010 Vehicle At Fault Accident Average = 0.6 <br /> o 2000 to 2006 Vehicle Accident Cost Average/Year = $20,000 <br /> o 2007 to 2010 Vehicle Accident Cost Average/Year = $7,700 <br /> ENVIRONMENTAL COMPLIANCE <br /> 0 Hydrocarbon Release Program <br />
