Immediately prepare a contingency plan for this <br />facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br />July 7, 2011. <br />49. The training records for the facility did not include job titles and descriptions. Facility personnel shall <br />complete a training program in accordance with Title 22 regulations. This program shall be directed by a <br />person trained in hazardous waste management procedures and shall include the following: <br />- hazardous waste management procedures, including contingency plan implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />The training program shall be completed by each employee within six months of hire, prior to <br />unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br />closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each position (job title) listed, including requisite skill, education, <br />qualifications, and duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately update the training records for this facility in accordance with Title 22 regulations. Submit a <br />copy of the updated training records the EHD by July 7, 2011. <br />85. Copies of universal waste disposal records were not found on site. A generator of universal waste <br />(alkaline batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol <br />cans) shall keep a record of each shipment of universal waste on site for at least three years.
