The designated operator failed to check and note the correct test dates for the secondary containment testing completed on December 18, 2014 on the July 2015 through July 2016 designated operator monthly inspection report. The designated operator listed January 28, 2014 instead of December 18, 2014 on the designated monthly inspection reports. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to sign the designated operator monthly inspection report for July 2, 2015 and the designated operator checked 'No'for"Each alarm for the previous month has been responded to appropriately"when there was no alarm reported from the previous month. The designated operator failed to complete items one through five on the designated operator monthly inspection report for July 11, 2016. During the monthly inspections, the designated operator shall check that all required testing and maintenance for the UST system have been completed. Ensure that designated operators perform monthly inspections at this facility are including all of the required information on the report. This is a repeat violation, Class Il. 211 CCR 2715(f) Designated operator employee training not performed or log not kept. The designated operator employee training for Daniel Mcllrath, Sharon Willey, David Ortiz, and Fernando Mendez did not include the HIRE DATE. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. A small amount of liquid was found in both the 87-product and 91-product spill containers and the liquid was removed by the service technician.
