The designated operator employee training and refresher training has not been performed since <br />prior to the shutdown of facility. The designated operator shall train facility employees for which he or <br />she is responsible in the proper operation and maintenance of the LIST system once every 12 months. <br />The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD. Within 30 days of restarting operations, submit training <br />records to EHD. <br />201 & 202. Monitoring and maintenance records not found on site or off-site at a readily available <br />location, if approved by EHD. It appears all monitoring and maintenance records that was left by the <br />evicted operator is no longer on site nor was EHD notified of records being off-site. Records were still on <br />site when a visit was made to the facility in December 2010. Monitoring and maintenance records shall <br />be maintained for at least 3 years, 6 and half years for cathodic protection maintenance records, 5 yras <br />for written performance claims pertaining to release detection systems, calibration and maintenance <br />records for such systems. Records of repairs, lining and upgrades shall be maintained for remaining life <br />of LIST. Located all these documents as they shall be made available, upon request within 36 hours to <br />EHD or State Water Board. <br />999. CCR 2712(d). Permit transfer of ownership has not been made. Within 30 days of the change in <br />ownership, new LIST owners must apply with EHD. Permits may be transferred to new UST if 1. new <br />owner does not change permit conditions, 2. transfer is registered through EHD, 3. state permit <br />application forms are completed to show changes.
