The 87 Tank 1 and 2 and the 91 Tank annular sensor failed to activate an audible and visual <br /> alarm due to the monitoring panel not being operational, therefore, the tank is not monitored. The <br /> interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm <br /> system. Immediately contact a properly licensed, trained, and certified contractor to replace the failed <br /> component with a functional component (LG 113-x listed, if applicable) under permit and inspection of <br /> the EHD within one business day. If unable to establish continuous monitoring of the UST system cease. <br /> operation and remove fuel from all the tanks. <br /> 13. All the under dispenser containment sensor and the STP sump sensors for the 87 Tank 1 and 2 and <br /> 91 Tank failed to activate an audible and visual alarm to ensure continuous monitoring of the tank <br /> system due to the monitoring panel not being operational. All monitoring equipment shall be maintained <br /> to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br /> leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the <br /> failed component with a functional component (LG 113-x listed, if applicable) and obtain a permit within <br /> 1 business day from the EHD. If the failed component can't be replaced immediately, there is a <br /> possibility that the 87 Tank 1 and 2 and 91 Tank UST system may be red tagged to prevent fuel inputs. <br /> 17. The 87 and 91 turbine electronic line leak detector failed to detect a leak due to the monitoring panel <br /> not being operational. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 <br /> psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed <br /> leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. <br /> If the failed leak detector can't be replaced immediately, there is a possibility that the 87 and 91 UST <br /> system may be red tagged to prevent fuel inputs. <br /> 21. Liquid was observed in the 87 fill sump on Tank 1 and 2. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the 87 fill sump on Tank 1 and 2 is maintained free of liquid. <br /> The technician removed the liquid and put it in the facility's hazardous waste storage container. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
