[ 2705 Country Club Blvd ] <br />The permit addendum to as listed on your "Additional Scope of Work" document has <br />been approved under the following conditions. <br />1. A conformation tank test must be conducted on the tank prior to the final <br />inspection. <br />2. All sumps and spill buckets will need to have a lake test or equivalent prior to <br />being put into operation. Schedule an appointment to be inspected. <br />3. Primary and secondary line that have been exposed will need to be pressure <br />tested, re- checked for slope, and soap tested prior to backfilling. Arrange an <br />Inspection. <br />4. Ensure this system meets one of the accepted overfill prevention options as listed <br />in Title 23 Division 3, Chapter 16, Sections 2635&2636. <br />5. Arrange an inspection to verify overfill prevention. <br />6.
