c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />And in addition <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />4. Any changes shall be submitted to SJC Environmental Health Department (EHD) <br />for approval prior to commencing work. All new submittals supersede the <br />previous ones. <br />5. The following information must be submitted within 30 days of the sample date: <br />a. Analytical results sent directly from a California certified laboratory or <br />this office WILL NOT accept them. <br />b. See page 3 of the permit application for required analytical tests. <br />6. No gasoline, diesel, or other hazardous material or waste can enter the <br />sewer/septic or storm water system. If it does it will be considered illegal <br />hazardous waste disposal and enforcement actions could follow. <br />7. All personnel involved in the removal of the existing pipes must fulfill <br />requirements of CCR, Title 8, Section 5192. <br />8. If any fuel is to remain in the underground storage tank, it must be continuously <br />monitored. <br />9. All piping, fittings, sumps and UDC's must be certified by an independent testing <br />organization and labeled as required by the organization. (UL, ULC, ANSI, API, <br />ASTM, etc., See California Code of Regulations, Title 23, Chapter 16 Table B for <br />complete list.). Pursuant to the Underwriters Laboratories Inc (UL) listings, only <br />those products bearing the UL mark will be considered listed. Any part/product <br />not marked with the UL label or another listed testing organization will be <br />rejected. <br />10. All installation and testing shall be in compliance with manufacturer's <br />instructions, Section 2635 and 2636 of CCR. <br />11. All piping must slope and be able to drain to a containment sump with an <br />audible/visual alarm. <br />12. All metal components in direct contact with the ground such as flexible <br />connectors, swing joints, fittings, and pumps shall be cathodically protected. <br />13. An integrity test must be performed in accordance with the manufacturer's <br />guidelines for the tank and the piping once the site is in operating condition (once <br />concrete is poured) and before the final inspection. A hard copy of the results <br />must be submitted to this office prior to operation. <br />14. An integrity test must be performed in accordance with the manufacturer's <br />guidelines for the tank and the piping six months after repair. A hard copy of the <br />results must be submitted to this office within 30 days of the test. <br />15. Submit the Best Management Practices to EHD for managing fuel spills and wash <br />down water at this site prior to the final inspection. <br />16. A "Certificate of Compliance for Underground Storage Tank Installation Form C" <br />shall be submitted by the owner or the owner's agent before this installation will <br />be considered complete by EHD. <br />17. An accurate and drawn -to -scale -as -built plan must be received by EHD prior to <br />scheduling the final inspection. <br />
