R <br />7 <br />JUN B 9 2016 <br />This information has been changed in CERS and submitted for review., <br />Failed to record or report unauthorized release in appropriate time frame. <br />On May 4, 2016 and on May 5, 2016, oily staining was noted on the floor (around the suction <br />pump) and pooling of oily liquid was noted inside the Biodiesel sump. Oily liquid was also <br />noted inside both biodiesel transition sumps on the above inspection dates. Any release from <br />the primary containment that does not escape the secondary containment and was cleaned <br />up within eight hours of being detected shall be recorded with the following information. <br />Operator's name and phone number <br />List of types, quantities, and concentrations of hazardous substances released <br />Description of actions taken to control and clean the release <br />Method and location of disposal of the released substances <br />Description of actions taken to repair and prevent future releases <br />Description of method used to reactivate interstitial monitoring after repair or <br />replacement of primary containment <br />Immediately record this release with the information required, and maintain it onsite with the <br />facility's monitoring records. Submit a copy of the recordable release statement to the EHD. <br />This is a repeat violation, Class 11. <br />This facility is continuously monitored and sensors are programmed for positive shutdown in <br />the event of a fuel alarm. Pilot has a Regional Maintenance Technician that checks all <br />sumps and UDCs for liquid on a weekly basis, in addition to verifying that all sensors are <br />locked in position and test boots are pulled back to allow for the earliest detection of a <br />primary piping failure. if product is discovered in secondary containment during a visual <br />inspection or in the event of a fuel alarm, the associated equipment is immediately disabled <br />and an emergency call is placed to the Pilot Helpdesk. A certified technician is dispatched, <br />the release is reported to the appropriate Agency, and any repairs are coordinated with the <br />CUPA. <br />As mentioned in the inspection report, oily liquid was discovered in the Biodiesel shed and in <br />the Biodiesel sumps, although the amount of liquid in the sumps was not enough to cause a <br />fuel alarm. Since this was discovered during the inspection, Jones Covey technicians were <br />able to remove the liquid from the affected areas. Technicians verified that all equipment was <br />functioning and free of leaks, and have subsequently checked this area to ensure the integrity <br />of the Biodiesel system. Please see the attached recordable release statement. <br />Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />The designated operator failed to check and note the correct test dates for the secondary <br />containment testing on all reviewed 2015-2016 designated operator monthly inspection <br />reports. The secondary containment test was listed on the reviewed DO reports as 7-27- <br />2014; however, the secondary containment testing for UDC -13, UDC 14-15, and UDC -16 <br />was conducted on 11-06-2013 and for the rest of the components on 8-19-14. During the <br />monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system <br />This is a Class 11 violation. <br />
