The designated operator monthly inspection reports will be corrected during the monthly <br />designated operator inspection on July 5, 2016. Pilot has reviewed the proper procedure for <br />documenting test dates and maintaining paperwork with the designated operator. <br />8. VPH monitoring of the interstitial spaces of the UST system is not maintained. <br />The "south" biodiesel transition sump had a hose and a valve with a gauge attached to the <br />end of the brine test port, when the valve was opened (and pressure released) the brine level <br />dropped significantly in the brine sensor holder and had to be replenished. The sensors <br />monitoring the liquid level in the interstitial space shall be maintained so that a breach in the <br />primary or secondary containment is detected before the hazardous substance is released <br />into the environment. The hose and the gauge were removed by the technician onsite at the <br />time of the inspection, ensure that all interstitial monitoring is being maintained as required. <br />This is a repeat violation, Class II. <br />As noted, this was corrected during the inspection. The hose and gauge on the test port was <br />not applied by Pilot personnel, and only certified testers and technicians access this sump. <br />Pilot will reiterate the importance of compliance with regulations to all third -party technicians <br />when responding to alarms or addressing maintenance items at this facility. <br />9. Monitoring system of the interstitial space of a double walled system not maintained. <br />The Diesel 3 tank annular sensor failed to activate an audible and visual alarm when tested <br />and, therefore, the tank is not monitored. The interstitial space shall be monitored with a <br />monitoring system connected to an audible and visual alarm system. The sensor was <br />replaced and retested during the inspection. A "UST Retrofit Verification with Inspector <br />Already Onsite" form has been completed and provided to the operator and contractor. This <br />was corrected on site. <br />This is a repeat violation, Class If <br />As noted, this was corrected during the inspection. <br />10. Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the <br />UDC. <br />The UDC -16 sump sensor cord was fraying and was taped (outside the "j" box) with electrical <br />tape. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. The sensor was replaced <br />and retested during the inspection. A "UST Retrofit Verification with Inspector Already <br />Onsite" form has been completed and provided to the operator and contractor. <br />This is a repeat violation, Class li. <br />As noted, this was corrected during the inspection. <br />11. Primary containment not tight (after June 2004). <br />Oily staining was noted on the floor of the biodiesel shed, around the suction pump and <br />pooling of oily liquid was noted inside the biodiesel shed sump, oily liquid was also noted <br />inside both biodiesel transitions sumps, indicating a leak in the primary piping.
