Emergency Response Action Plan Version :27.10.4 <br /> Evacuation Routes and Alternative Routes of Evacuation <br /> The primary evacuation route is through the entrance and/or exit gate heading East of the <br /> terminal to Nustar facility on Navy Drive. Use evacuation maps to plan evacuation course. <br /> When evacuation procedures have been initiated, the QI or Alternate will obtain an <br /> attendance roster and the daily visitor log to verify that all employees and visitors have <br /> evacuated. In the event that employees have evacuated to multiple assembly areas, <br /> evacuation checks can also be accomplished using facility radios. <br /> Transportation of Injured Personnel <br /> Injured personnel will be transported to Dameron Hospital in the city of Stockton,CA located <br /> approximately 5.0 miles southwest of the Facility. From Facility head east on Navy Dr toward <br /> Fyffe Ave 0.2 miles. Continue on Navy Dr. for 4.0 miles and take Ort J. Lofthus Fwy and Exit <br /> 65B to Acacia St. Turn left to stay on Navy Dr about 0.5 miles and to stay on Navy Dr take a <br /> right, stay on Navy drive approximately 1.0 mile. Turn left onto Ort J. Lofthus Fwy for <br /> approximately 1.0. Use the right 2 lanes to take exit 65B to merge onto I-5 N toward <br /> Sacramento. Then take exit 473 toward Pershing Avenue. Continue straight onto N Pershing <br /> Ave for 492 feet. Turn right onto Acacia St for 0.7 miles and turn left onto N Lincoln Street, <br /> travel 108 feet and then take the left onto Acacia Street to the entrance of the hospital. <br /> • First Aid Supplies/Kits are located in the Operators office. <br /> .
