STATE OF CALIFORNIA 0 0 Page 6 <br />DEPARTMENT OF GENERAL SERVICES <br />PROCUREMENT DIVISION <br />Contract (Non -Mandatory) 1-07-65-54-A <br />J) All participating MMCAP members will receive a Disposal Manifest and Certificate of <br />Destruction from the Contractor within ten business days via US Mail as receipt that the <br />merchandise was received and properly destroyed. These reports will also be available <br />in the Internet via the Vendor's GRX Extranet. <br />K) Participating MMCAP members disposable merchandise is determined to be waste after <br />the Contractor receives the products. At that time, the Contractor becomes the waste <br />generator, not MMCAP or its members. All waste disposal is manifested to the <br />incinerator under the Medical Waste Tracking Act. <br />Q Contractor must perform In -Date Aging and Recall Management Programs, for no <br />additional fee. <br />M) Reports: Upon the request of MMCAP or a MMCAP member, the Contractor must <br />supply an electronic or hard copy of reports. Examples are: <br />• Vendor Summary Report: The report will summarize the total amounts invoiced to all <br />manufacturers, as well as the percentages on non -creditable values to the total <br />return. <br />• Vendor Policy Guide: This report will be a summary of all the manufacturers' general <br />® return goods policy and is available to all participating MMCAP members who <br />prepare their own returns. <br />• Inventory Control Report: This report will total all like NDC/UPC numbers having full <br />quantities. <br />• Reason Code Summary Report: This report will pinpoint excessive items not eligible <br />for credit. <br />The Contractor will maintain its 140 custom reports in its portfolio. If a new report and/or <br />modification is required by MMCAP, the Contractor will develop it at no additional <br />charge. <br />Reports will be accessible through the Contractor's ExtraNET <br />(www.guaranteedreturns.com), or will be e-mailed by the Contractor to the requesting <br />party in ASCII or Comma Delimited formats. <br />N) Web page: The Contractor must provide a user-friendly ExtraNet web page <br />(www.quaranteedreturns.com) which may be used by participating MMCAP members to <br />retrieve all return goods data and current activities. <br />8. DGS' RESPONSIBILITIES <br />® A) DGS will advertise and market this contract to state agencies. <br />B) DGS will provide MMCAP facility membership form, will authorize eligibility and return <br />completed forms to MMCAP <br />C) DGS will manage this contract. <br />
