Attachment A <br />Scope of Service -6- Contract Number MLT97000 <br />MEDICAL WASTE HANDLING SERVICES continued <br />removal, repackaging, loading, transportation and disposal of all medical waste <br />from the designated locations at the institution to the final disposition by <br />incineration or steam sterilization. <br />B. Prime Contractor shall be responsible for all work performed under the contract. <br />Prime Contractor shall be a Registered Hazardous Waste Hauler. If a sub- <br />contractor is utilized to perform any work in connection with this contract, the <br />Contractor must provide the name, address, phone number, contact person, and <br />Environmental Protection Agency (EPA) identification number of the sub- <br />contractor when submitting the bid proposal. All changes in subcontractual <br />relationship during the term of this contract must be received prior approval from <br />the State. If the medical waste treatment/disposal is sub -contracted, the <br />Contractor must submit with his bid proposal, a copy of the Public Utilities <br />Commission Operating Authority Permit for Property Transportation issued to the <br />Contractor/hauler. <br />C. Contractor shall obtain and maintain from Federal, State and local regulatory <br />agencies all required permits, licenses, registrations and other documentation for <br />the collection, removal, transportation, and disposal of medical wastes. All <br />permits, licenses, registrations and other documentation shall be current and in <br />good standing. Contractor shall obtain copies of all required permits, licenses, <br />registrations and other documentation for any sub -contractors utilized in <br />connection with this contract. <br />D. Contractor shall be in good standing with the State of California, Department of <br />Health Services, Environmental Management Division. <br />E. Contractor shall inspect medical waste packaged by the institution personnel and <br />make necessary corrections to ensure compliance with all the regulations and <br />a designated waste disposal site requirements. <br />F. Contractor's drivers and handlers of medical wastes shall: <br />1. be equipped with disinfectants which meet the requirements as defined by <br />and listed in HSC, Article 8, Section 25084 in the event of medical waste <br />spills; <br />2. be trained in the spill clean-up, decontamination and disinfection <br />procedures; <br />
