Attachment A <br />Scope of Service -8- Contract Number MLT97000 <br />MEDICAL WASTE HANDLING SERVICES (continued) <br />3. inspect all designated medical waste containers to ensure compliance <br />with HSC, Article 8, commencing with Section 25080. The institution will <br />conduct periodic inspection of the storage bins and lockers and its <br />immediate area. The Contractor shall immediately correct all insanitary <br />conditions or deficiencies as noted by institution inspections; <br />4. provide a completed medical waste tracking document at the time of each <br />pick-up, pursuant to HSC, Article 6, Section 25063. The document shall <br />be properly executed, signed and dated by the State Project Coordinator <br />or designee and the waste hauler at each of the institution locations. Only <br />the State's Project Coordinator or designee shall review and sign the <br />original manifest (for billing authorization) prior to shipment of medical <br />waste. Within fifteen (1) calendar days after medical waste pick up, a <br />copy of the manifest with the disposal site information section completed <br />shall be furnished to the State Project Coordinator. Invoices will be <br />processed upon receipt by the State Project Coordinator of a copy of the <br />treatment, storage and disposal (TSD) facility manifest, establishing <br />acceptance of the waste at the disposal site, and the Certificate of <br />Disposal from the ultimate disposal facility. <br />5. Contractor shall provide an official scale, approved by the State <br />Department of Food and Agriculture Measurement Standards Division, to <br />weigh medical waste. Contractor shall be responsible for weighing all <br />medical waste at the time of pickup. Weights shall be included on the <br />invoice submitted for payment. <br />I. Contractor shall maintain true and correct records for each service performed <br />and all transactions related thereto and shall retain all such records in <br />compliance with all applicable federal, state and local laws, ordinances, rules, <br />and regulations. <br />J. Contractor shall provide the institution with copies of all documents relating to <br />each service performed, medical waste tracking documents and recordkeeping, <br />certificate(s) of destruction, and all other related licenses and permits required by <br />the appropriate governmental authorities as per HSC, Article 6, Section 25063. <br />K. All completed medical waste certificates of destruction with the corresponding <br />invoices shall be submitted to the institution. <br />
