Attachment A <br />Scope of Service -10- Contract Number MLT97000 <br />A All waste containers shall be in accordance with requirements set forth in HCS, <br />Article 8, commencing with § 25080. <br />B. The institution shall place all sharps (i.e., syringes and needles) in sharps <br />containers; all biohazardous waste shall be placed in biohazardous bags by the <br />institution prior to the scheduled pickup. Sharps containers may be placed in <br />biohazardous bags. Containers will be made available for exchange. <br />C. Contractor shall provide disposable plastic biohazardous bag, each bag shall be <br />red in color, and shall be labeled with the word's "BIOHAZARDOUS WASTE" or <br />with the international biohazard symbol and the word "BIOHAZARD" THE RED <br />BAG IMPACT RESISTANCE TEST SHALL BE CERTIFIED BY THE BAG <br />MANUFACTURER. <br />NOTE: Some Institutions may provide a "Biohazardous bag" which will meet the <br />definition under HSC, Article 2, Section 25020.2. For institutions that provide the", <br />bags, all references to the contractor providing the bags will not apply. <br />All full sharps containers shall be taped closed to preclude loss of contents. <br />Stored sharps containers ready for disposal will not be stored by the institution <br />for more than seven days unless the institution generates less than 20 pounds of <br />medical waste per month. Sharp containers shall be labeled with the words <br />"SHARPS WASTE" or with the international biohazard symbol and the word <br />"BIOHAZARD". <br />Storage, handling, and transportation containers shall be labeled with the words <br />"BIOHAZARDOUS WASTE" or with the international biohazard symbol and the <br />word "BIOHAZARD", on the lid and on the sides so as to be visible from any <br />lateral direction. <br />All designated storage areas shall be marked with warning signs on or adjacent <br />to the exterior of entry doors, or gates and lids. The wording of warning signs <br />shall be in English, "CAUTION - BIOHAZARDOUS WASTE STORAGE AREA - <br />UNAUTHORIZED PERSONS KEEP OUT", and in Spanish, "CUIDADO - ZONA <br />DE RESIDUOS - BIOLOGICOS PELIGROSOS - PROHIBIDA LA ENTRADA A <br />PERSONAS NO AUTORIZADAS", or other languages as determined by the <br />institution. Warning signs shall be readily legible during daylight from a distance <br />of at least 25 feet. <br />
