[A <br />Resurrection Carpet Cleaning & Janitorial Services <br />CDCR Scope of Work <br />FINGERPRINTING AND CLEARANCE BY THE STATE <br />Agreement Number DVIO8009 <br />Exhibit A <br />Any and all employees providingthis his service shall be subject to fingerprinting and <br />clearance by the State through the Department of Justice, Bureau of Criminal Identification <br />and Information before being permitted access to the premises. Until employees are <br />cleared, the Contractor may not be allowed to perform services. <br />3. Deferring Service <br />Services shall be provided upon request and at the direction of the Health Care Manager, <br />Chief Medical Officer (HCM/CMO) or designee. CDCR shall defer services if the service <br />is non-essential or under high security risk. <br />4. General Duties (All Areas) <br />The requirements below are provide as general guidelines for janitorial services, but are <br />not limited in scope by these duties. The work performed is expected to be consistent in <br />that the Health Care Services areas of DVI will be kept clean from both a technical and <br />aesthetic perspective. <br />The Contractor will be responsible for ensuring that all employees have Universal <br />Precautions to include, Personal Protective Equipment, Bio -hazardous Waste Handling <br />and Infectious Linen, and Infection Control Practices. The Contractor shall ensure that <br />high risk areas are to be assigned to the same personnel on a routine basis. <br />All Medical Facilities serviced by the Contractor are to be cleaned in accordance with Title <br />22 guidelines, California Code of Regulations, California Rehabilitation Control <br />Regulations, Division 5, Chapter 12, §79841 and §79843. <br />All blood spills must be handled using Universal Precaution Requirements and the <br />necessary Protective Personnel Equipment must be implemented. <br />Biohazard bags in all areas are to be emptied and removed nightly. Replace biohazard <br />bags when full, or causing an odor. Wipe out containers with germicidal solution and put <br />in new red bag. Place bagged contents in appropriate biohazard container and seal the <br />container when full. Contractor shall not transfer biohazard materials from one container <br />to another, or reuse biohazard bags. The biohazard storage area shall be kept orderly, <br />clean and free from odor. <br />After discharge/treatment of a patient, the bed, bedding, and room furnishing used by said <br />patient shall be (terminally) cleaned. <br />A. Services To Be Provided Daily: <br />• All trash receptacles shall be emptied; trash and paper shall be bagged <br />and transferred to trash container/dumpster for waste disposal. Trash <br />liners to be replaced daily. <br />• Clean, sanitize and polish all drinking fountains. <br />• All horizontal and vertical surfaces which collect dust will be dusted with <br />treated cloth. <br />• All inmate holding room furniture shall be wiped down and cleaned with <br />sanitizing solution. <br />S►a <br />
