Field Operations Policy and Procedures <br /> • Taking keepsake prints <br /> • Cremation <br /> • Family visitation or visual identifications <br /> • Final identification prior to disposition <br /> • Transfer to/from cemetery, crematory, airport, church or other location <br /> • Transfer within a location (i.e. viewing room to chapel) <br /> • Transfer between Company representatives <br /> Procedure. <br /> At every stage that a Company representative handles a decedent(human or cremated remains): <br /> I. Compare the name on all paperwork with the name on the white identification band. <br /> 2. If the name on all of the paperwork matches, complete the Chain of Custody <br /> Identification Confirmation Form (FIN-CS004) and proceed. <br /> 3. If any information on the paperwork or the white identification band does not match, stop <br /> all processes, contact the Location Manager and resolve the inconsistences before <br /> proceeding. <br /> 4. If a transfer from location to location occurs after hours and no Company representative <br /> is available, a Company representative signs the Chain of Custody and Identification <br /> Confirmation Form (FIN-CS004)promptly the following business day. <br /> 5. For shipping cases, confirm the decedent's identification by verifying the name on the <br /> casket/shipping container identification tag matches the accompanying paperwork as the <br /> white identification band on the decedent will not be visible. <br /> 6. Retain the Chain of Custody and Identification Confirmation Form (FIN-CS004) with the <br /> decedent throughout the handling process up to final disposition. <br /> 3.6 Decedent Check-In <br /> Policy.
